CANDIDATE REGISTRATION AND CONSENT

Completed with every candidate before the first assignment · Regulations 14, 19, 21 and 22, Conduct Regulations 2003; UK GDPR Articles 6, 9 and 13

This is the record of what you told us and what you agreed to, on the day you agreed to it. You get a copy. Everything on it that is optional is marked optional, and leaving an optional box empty costs you nothing at all: it will not affect whether we put you forward for work, and if anybody ever suggests it did, tell us.

1. You

Full name: _______________________________________________
Address: _________________________________________________________________________
Telephone: ____________________   Email: ____________________________________
Date of birth: ______________   National Insurance number: ____________________

Emergency contact, name and number: ____________________________________________

The emergency contact is for one purpose: if something happens to you on a shift. It is not used for anything else and it is never given to a client.

2. Right to work

Checked before your first shift, on everybody, without exception. What to bring is on our website at www.hiringuk.co.uk/what-you-need-to-work.

Share code: ____________________   Checked on: ______________   By: ______________
Or document seen: ______________________________________   Expiry if any: ______________

3. The work you are looking for

Roles: ____________________________________________________________________
Areas you can reach, and how you travel: ___________________________________________
Days and shifts you can do: ________________________________________________________
Anything you cannot do, and you do not have to say why: _____________________________

4. Drivers only

Licence categories: ____________________   Licence number: ____________________
Endorsements and points, with dates: _______________________________________________
Driver CPC card expiry: ______________   Digital tachograph card expiry: ______________
ADR classes and expiry, if any: ____________________________________________________

Other work as a mobile worker. The 48 hour average covers every hour you drive for anybody, so we have to ask. The full declaration is a separate form and it is not optional.

5. Tickets and training

Forklift categories and the date on the certificate: _________________________________
Other cards (CSCS, first aid, food hygiene, SIA): ____________________________________

Categories matter more than the card. A counterbalance certificate does not cover a reach truck, and we will not send you somewhere expecting one when you hold the other.

6. References

Regulation 22 asks us to take up references for some roles, and we take two for any driving job. We ask you first and we do not contact anybody before you say yes.

1. Name, company, relationship, contact: _____________________________________________
2. Name, company, relationship, contact: _____________________________________________

☐   I agree to HIRINGUK LTD contacting the people above for a reference.
☐   Please ask me before contacting my current employer.

7. What we do with all this, and what we never do

  • Our lawful basis is the contract between us and our legitimate interest in finding you work, not your consent, because consent you could withdraw halfway through a compliance obligation would not be a basis we could run on. The optional parts in section 8 ARE consent, and they are separate for that reason.
  • We show a client what they need to decide, and no more. Your name, the experience relevant to that job, and the outcome of the checks. Not your address, not your date of birth, not your medical, not a DBS certificate.
  • We never sell access to a database of candidates, and this is not a capacity question that changes when we are bigger. It is on our website under what we will never do.
  • How long. A year from the last time we provided you a service, because regulation 29 of the Conduct Regulations requires at least that, and then the periods in our retention schedule. You can ask for a copy of everything we hold, or ask us to erase it, at dpo@hiringuk.co.uk.

8. Optional, and genuinely optional

☐   Keep my CV on file so you can tell me about other jobs. Erased after twelve months unless I confirm, and I can stop it with one click from any email.

☐   Send me job alerts by email. At most once a week, one click to stop.

Ticking neither of these has no effect on anything. The wording you agreed to, its version, and the date and time are stored with the record.

9. Your declaration

The information above is true as far as I know. I will tell HIRINGUK LTD if any of it changes, particularly my licence, my cards, my endorsements or my other work.

Signed: ______________________________________    Date: ______________

Taken by: ______________________________________    Copy given to candidate: Yes / No

You are never charged for anything. Not for registering, not for a check, not for a card, not for finding you work. That is section 6 of the Employment Agencies Act 1973 and it is also our own rule.
